From Ancient Greek κοκκύμηλον (kokkúmēlon), from κόκκῠξ (kókkux, “cuckoo”) + μῆλον (mêlon, “apple, any fruit from a tree”). The ending in -i could be a consequence of dissimilation from Sicilian meli, from Latin mel. Compare also Sicilian millicuccu, Greek Κουκουμέλης (Koukoumélis).
